Responsibilities:
As a Social Worker our Adult Day Center, supporting the mission will include the following
Responsibilities:
Interviewing patients and family members to determine needs and concerns while completing an initial assessment.
Providing and/or arranging for individual, family, and group counseling reference to psychological, social, financial, legal, vocational, and educational needs of patient.
Initiate the plan of care and make necessary revisions as patient status and needs change.
Use health assessment data to cross reference and determine patient needs.
Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Assist patient with community resources for housing, shopping, clothing, etc.
Refer to and/or developing support groups and educational programs for caregivers and patients.
Arranging and/ or providing crisis intervention Providing family outreach Providing in-service training to staff on patient/caregiver psychological needs Participating in facility's quality improvement program Participating in professional organizations and seminars Requirements:
Current and Valid Social Work license certification in New Jersey 1 year experience in adult day care is preferred Job Types:
M-F, 35-40hrs/wk Salary:
